Acceptance of Terms By using GlowWell Health services, you agree to these Terms of Service. If you do not agree, please discontinue use of our services.
Description of Services GlowWell Health provides telehealth services including menopause management and weight loss consultations through licensed healthcare providers via secure video technology.
Eligibility
- You must be 18 years or older
- You must be located in TX, MD, NY, FL, CO, or AZ during consultations
- You must provide accurate medical and personal information
User Responsibilities
- Provide accurate and complete information
- Attend scheduled appointments or cancel with appropriate notice
- Follow prescribed treatment plans as directed
- Maintain confidentiality of your account credentials
Payment Terms
- Payment is due at time of service
- We operate on a cash-pay basis
- Refunds are subject to our refund policy
- You are responsible for any bank or payment processing fees
Limitations of Service
- Telehealth services are not appropriate for medical emergencies
- Some conditions require in-person evaluation
- We cannot guarantee treatment outcomes
- Services are limited to the states where our providers are licensed
Cancellation Policy
- Appointments must be cancelled at least 24 hours in advance
- Late cancellations may incur fees
- Either party may terminate the provider-patient relationship with appropriate notice
Liability Limitations Our liability is limited to the maximum extent permitted by law. We are not liable for technical failures, internet connectivity issues, or outcomes beyond our reasonable control.
Governing Law These terms are governed by the laws of Texas.
